Registered Nurse (RN) - Part Time

Start as contingent or part time. The RN will have an option to work their way into a full-time caseload.

Responsibilities Include:
 
  • Ensure Start of Care OASIS is accurate and in compliance with quality and safe delivery of home health services.
  • Establish a Nursing Plan of Care with prescribed treatments for patients in accordance with physician’s orders and in compliance with agency policies/procedures, state, and federal regulations.
  • Evaluate and regularly re-evaluate the nursing needs of the patient. Initiate, develop, implement, and make necessary revisions to the patient’s individualized plan of care. Address all problems in the plan of care.
  • Observe signs and symptoms and report significant findings or changes in the patient’s condition to the physician and Clinical Manager.
  • Teach and supervise the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition.
  • Document all care in the electronic medical record.

Requirements:
 
  • Active Michigan Registered Nurse (RN) license
  • Current BLS Certification
  • Valid Driver’s License and auto insurance in your name as a driver

Home Health Aide (HHA) – Part Time

Start as contingent or part time. The HHA will have an option to work their way into a full-time caseload.

Responsibilities Include:
 
  • Provide care in accordance with written instructions from nurses and therapists.
  • Assist patients with personal care activities, including bathing, grooming, dressing, meal preparation, medication reminders, and light housekeeping, etc.
  • Assist with simple exercises, transfers, and assist with ambulation.
  • Report any patient changes and/or concerns to the nurse or therapist.
  • Support patients in maintaining their independence and dignity.
  • Document all care in the electronic medical record.

Requirements:
 
  • Active Michigan CNA certification
  • Current BLS Certification
  • Valid Driver’s License and auto insurance in your name as a driver

Physical Therapist (PT) – Per Diem

Start as contingent or per diem. The PT will have an option to work their way into a full-time caseload.

Responsibilities Include:
 
  • Direct the assessment, treatment, planning, implementation, and education to address the physical therapy needs of patients.
  • Develop and implement a physical therapy treatment plan care based upon evaluation of patient status and in coordination with the patient and physician.
  • Provide direct patient care, evaluate functional needs and outcomes, consult with other members of the care team as necessary, reassesses patient status and adjust the plan of care as needed to ensure optimal outcomes.
  • Document all care in the electronic medical record.

Requirements:
 
  • Active Michigan Physical Therapy (PT) license
  • Current BLS Certification
  • Valid Driver’s License and auto insurance in your name as a driver

Physical Therapy Assistant (PTA) – Per Diem

Start as contingent or per diem. The PTA will have an option to work their way into a full-time caseload.

Responsibilities Include:
 
  • Work closely and under the direction of the physical therapist, treating patients with medical problems or other conditions that limit their ability to move or perform functional activities.
  • Implement selected components of the plan of care and interventions.
  • Instruct patients and their families in the treatment plan and in the use of prosthetic, orthotic, and assistive devices.
  • Maintain communication with the Physical Therapist /Nurse regarding the patient’s response to treatment and/or any changes of the patient’s condition.
  • Document all care in the electronic medical record.

Requirements:
 
  • Active Michigan Physical Therapy Assistant (PTA) license
  • Current BLS Certification
  • Valid Driver’s License and auto insurance in your name as a driver
