Home health nursing is a nursing specialty in which nurses provide multidimensional home care to patients of all ages.
Our experienced nurses perform in-depth home health assessments to understand the unique needs of our patients. After carefully developing mutual goals with the patient and physician, a program is created based on a variety of treatments. Our nurses work together with other healthcare providers to promote and maintain the health and well-being of our patients, providing support and compassionate care in the comfort of the home.
Our nurses provide a range of services, including but not limited to acute or chronic disease management and patient education, monitoring vital signs, set-up and administration and oversight of medications, blood sugar testing, and wound care. We implement the patient’s plan of care in compliance with agency, state, and governmental regulations.

Home Health Aide (HHA)
Certified Nurse Assistants (CNAs) Providing Personalized Care and Monitoring for Your Loved One's Well-being.
Our Home Health Aides are Certified Nurse Assistants (CNAs) who provide care with written instructions from our professional staff of RNs. HHAs perform personal care activities such as bathing, hair care, oral hygiene, and dressing. They can facilitate simple exercises and transfers according to the plan of care. While carefully monitoring the health of your loved one, our HHAs report any changes in condition to the Registered Nurse.

Physical Therapy (PT)

Our home-based Physical Therapists work closely with patients to develop an individualized plan to improve their ability to move, manage pain and prevent further disability. They help patients improve their strength, balance, and coordination through home-based exercise, manual therapy, and other treatment modalities. They are an important part of preventive care, rehabilitation, and treatment for patients with acute or chronic conditions, illnesses, or injuries.

Occupational Therapy (OT)
Occupational therapy helps individuals develop or regain skills
necessary for daily living and work.
Our home-based Occupational Therapists help patients meet goals to develop, recover, improve, and maintain skills needed for daily living and working. Occupational therapists offer a comprehensive perspective to help patients adapt to their environment and safely and successfully return to their prior level of functioning.

Speech and Language Therapy (SLT)

A Licensed Master Social Worker (LMSW) in home health care plays a critical role in supporting patients and families by providing psychosocial assessments, counseling, and resource coordination. Our social workers are available to conference with the patient and the family regarding all adjustments to illness, financial concerns and resource planning. Our Social Workers provide short-term counseling to you and your family as well.
